Credit Card on File (CCOF) 

Live Oak Dermatology has implemented a policy that enables the secure storage of your credit card information on file consistent with our financial policy. The credit card on file is stored securely by the merchant processing company.

Copays/Co-insurance/deductible: Co-pays and co-insurance are due at time of service. You may still make payments by check, cash, or on our website with a different credit card.

Outstanding Balance: If your insurance provider has paid their portion of your bill, and a balance is still owed. Live Oak Dermatology will contact you. If balance is not paid in full within 3 weeks of the statement notice, the balance will be charged to your credit card on file. A receipt for the charge will be sent to you. This does not compromise your ability to dispute a charge or question your insurance company's determination of payment.

The cardholder may also revoke this consent at any time in writing. We will collect estimated cost bill at time of service consistent with our financial policy.
